FORMS AND LETTERS

Routine school or work excuses are available upon request at the end of your appointment.  If time permits, brief forms (less than 5 minutes) for school (i.e. OHI) may be completed during your allotted appointment time and there will be no additional charge. Please notify me at the beginning of the appointment if these forms are required. Longer forms and letters will be done outside of appointment time and the fee will be based on the time involved to complete this service.  Please see below.

Simple (less than 5 minutes) No Charge

Moderate (5-15 minutes) $75.00

Lengthy (15-30 minutes) $175.00

Complex (30-1 hour) $350.00/Hour 

Please note that Dr. Stahl does not complete paperwork or provide letters regarding custody/divorce issues or disability evaluations.
